You may download a free Vetpath Vet Lab Requisition Form in Microsoft WORD format by Clicking on the link below in blue, and then selecting either save, save link, or save target as depending on the browser. We would suggest making a sub-directory called C:\vetpath, to save it in, or you can take the default, like "MyDocuments".

This will download a WORD FILE "VLI_REQ.DOC", which you must save in, e.g.,  "C:\vetpath".

The download is approximately 40 KB and should take less than a minute to download over a 56K modem and a good connection; just a few seconds with a DSL line, etc. . . 

Detailed downloading instructions:

1. When the screen pops up asking to OPEN or SAVE, choose SAVE. Save in a suitable directory.

2. When the download finishes, you can either "CLOSE" or click on OPEN on the screen, and if you have Microsoft WORD, or WordPerfect, WORD Viewer, or another compatible word processor, then the document will open. You will then print a copy, fill out the required fields, and include this form with your specimen that you ship to Vetpath Lab.
